Final narrative

An employee was working behind a stack of steel I-beams (stacked 2 columns deep and 4 levels high on the floor) while a forklift was pushing the stack. When the forklift took off the second-to-last beam, the last one was pushed over, causing it to fall to the ground and strike the employee. The employee sustained a fracture to the right leg.

An employee was walking around a flatbed that was being unloaded by a forklift. The employee was placing dunnage on the floor while the forklift removed a double-stack pallet of foam. The dunnage from the double stack pallet became entangled with the adjacent pallet, causing the top pallet of the double-stack to fall and strike the employee. The employee sustained a dislocated ankle and fractured lower tibia and was hospitalized.

The injured employee was assisting with removing a metal pole used for a basketball hoop. Employees had cut down the pole to almost ground level. They put a tow rope around the remaining pole and hooked it to the hitch of a pickup truck to pull it out of the ground. The injured employee was standing by the pole to guide the rope. When the pole came out of the ground, it swung up and struck the injured employee on the left side above his waist and the right side of his head. He was knocked to the ground and his left calf was scraped. The injured employee was hospitalized with a fractured rib and lacerations. He required surgery.
