An employee was part of a four-person crew that was removing a form after a concrete pour was set. A crane tried to pull the form off a concrete column, and the wire rope guideline hung up on something. The employee tried to release the guideline; the employee's right hand was caught between the form rebar and the guideline. The employee suffered a pinch-type amputation of the right index, middle, ring, and little fingers.
