On July 9, 2019, an employee was being trained to calibrate an oxygen test bench. He was lying on the floor under the bench during a simulated repair when a 40-pound counterweight's cable broke loose. The counterweight fell from 3 to 4 feet above the floor onto his head. He suffered a broken eye socket and possible skull fracture and was hospitalized.
