An employee was using a knife to remove a solid gasket that was attached to the manway of an oil/gas/water separator. A vacuum on the separator was being used to draw the remaining contents out of the bottom of the tank to a vacuum truck. The vacuum pulled the employee forward, causing a traumatic head injury, an injury to a neck vertebra, several damaged/broken ribs, and a collapsed lung.
