An employee climbed a fixed ladder on the glass crusher to perform a product purge. The employee pulled the handle on the diverter box and the handle snapped and broke. The employee lost his balance and grabbed the side of the metal diverter box with his right hand. The diverter plate fell on his right hand and crushed it against the side of the metal box. The employee sustained a laceration to the index finger which developed an infection and required medical amputation of the intermediate and distal phalanges.
