Final narrative
A temporary employee was using a shovel to clean out spilled material (crushed glass) from an access panel in the bottom of a vertical bucket conveyor/elevator that was clogged. As the material was removed, a bucket in the elevator shifted downward and fell on top of the employee's right hand. The employee's hand was pinned against the material at the bottom of the elevator, resulting in a severe contusion and swelling that required surgery.
