Final narrative
An employee was installing valves on a vessel. He squatted down and grabbed a mid-rail wire rope guardrail. The guardrail loosened and the employee lost balance. The employee then slipped between guardrails and fell approximately 9 feet to a lower deck. The employee sustained a left hip fracture that required surgery.
