A five-man crew had been subcontracted by Laredo Petroleum to replace the fire tubes on a heater treater and clean the inside of the tank. Three employees with Dorado Construction Group were contracted as a roustabout crew to clean the tank and replace the fire tubes, one employee of Mulholland Energy Services was contracted to operate the vacuum truck and remove the sludge from the tank, and one employee of Permian Service Co., LLC was hired to operate a hot oil truck to provide water to the roustabout crew for cleaning. At approximately 1:00 p.m. the employees had completed pulling out the old fire tube and had begun the process of cleaning the inside of the tank from the open hatch when an explosion occurred. Three employees were hospitalized for second and third degree burns to their arms and face.

An employee was cleaning leaking plastic from an injection nozzle. As the employee was lifting and dislodging plastic, the nozzle shifted and burst apart. The nozzle struck the employee's mouth and jaw and burned the employee. They also sustained a jaw fracture.

An employee turned on a pump to unload a truck. There was an explosion that caused burns to the employee's hand and face.

Tensaw Land & Timber Company, Inc.

An employee was fueling a tractor with diesel when there was an explosion and a fire started. The employee sustained third-degree burns to 32% of his body, chest, and arms.

An employee was torching materials to free a jam in a baler machine when an explosion occurred, resulting in second-degree burns to their left arm and leg that required hospitalization.

An employee was welding an oil pipe and installing a gauge on the pipe of a drilling rig. As he cut into it with a torch, an explosion occurred. The shockwave of the explosion caused the employee to sustain a concussion and multiple broken bones.
