An employee was working on a stand-alone rig. The employee had just broken off the tubing swivel with the tongs and was preparing to make the last connection. He placed his left hand near the opening of the tongs to push the tongs off the connection, but the Kelly hose was caught on the back of the tongs. The employee used his right hand to push the Kelly hose out of the way. The hose caught the operating handle and engaged the tongs causing the amputation of his left index and middle fingers and an oblique fracture of his left thumb.
