Final narrative
An employee was placing a rug into the mandrel opening to be rolled and wrapped when she experienced an issue getting the rug corner into the slot opening. At the same time, a co-worker came to her left to place a second rug into position to be rolled and wrapped on the same cycle. The switch that clamps the ends of the rugs was activated and the employee's right middle fingertip was pinched, resulting in a partial amputation.
