Final narrative
On March 2, 2022, at approximately 1:43 p.m., three employees were attempting to lower a 42 inch (in diameter) concrete pipe into an approximately 4 foot deep trench located along a city street. The crew had lifted the concrete sewer pipe by placing an eye of a 1/2 inch diameter rope sling through an approximately 2 inch lifting hole located in the center of the 42" concrete pipe, and placing a 1 1/2 inch bar through the eye of the sling (inside the pipe). The 1/2 wire rope sling was approximately 5 feet in length. The wire rope sling broke below the eye that was placed through the pipe, causing the pipe to fall on the injured employee and fracture both legs.
