Final narrative
An employee was in a 20-foot deep trench box, working to lay sanitary water pipe. The trench boxes were rigged with a 4-way chain rigging device. One of the legs of the rigging broke while a trench box was being moved. The chain struck the employee's right forearm, breaking it.
