An employee was performing preventative maintenance on a commercial-sized ironer. After oiling the chain and sprocket, he reached into the gear box with a rag to clean residual oil from the chain. The chain pulled in the rag and his left hand. His index finger was caught in the chain and sprocket, which amputated the fingertip.
