The injured employee was observing another employee utilizing a forklift to lift a tool off a testing cart. After the forklift lifted the took, the driver began to back up. The forklift rolled over an uneven surface which cased the lifting strap holding the tool to disengage from the forklift forks and drop to the ground. As it dropped, it contacted an additional tool stand near where the injured employee was located. When the injured employee noticed the tool stand falling over, he put his left hand out and it was caught between the tool and the stand resulting in amputation of the middle and ring fingertips.
