An employee was setting up a four-foot part in a lathe. The part had a square shaft with two-inch studs protruding from it. As the employee was adjusting the steady rest bolts, their shirt sleeve became caught on the rotating part of the lathe, resulting in a soft tissue injury to the upper left arm that required surgery.
