An employee was helping to move a drainage structure across a work zone. The drainage structure was rigged to an excavator and the hook became caught on the linkage of the drainage structure. The hydraulic bucket cylinder continued to move, causing the hook to stretch and the clevis to break. The chain snapped and the clevis struck the employee, resulting in a right shoulder fracture.
