On December 11, 2022, employees were drilling a new well using a drill rig. The injured employee had stepped off the drill rig platform to inspect the surrounding areas and ensure that the site was operating correctly. A 20-foot long steel drill rod struck the drill rig while being inserted into the carousel, which caused the rod to strike the injured employee in the back. The employee then fell face down into the fresh mud. The tube then began to raise and collide with the drill, causing it to come loose and fall on top of the injured employee. The employee sustained fractures of the right scapula, multiple ribs on the right side, the C7 vertebra, and T6-T7 vertebrae. The employee also sustained a brachial plexus avulsion injury.
