Final narrative
On February 8, 2023, two employees were installing a cascade attachment (with four thin forks that extended out and could pull together to form two forks) on a forklift. After fixing the installation of the hoses on the forklift, the employees were checking to ensure that they would not be pinched when the attachment was activated. While one of them was checking the placement of the hoses, the attachment was activated. The forks pulled together and crushed his right ring finger, causing a laceration to the second knuckle; the fingertip was medically amputated.
