Final narrative
An employee was placing a piece of plywood on a table saw. As he was sliding the plywood off a cart and onto the table saw, the plywood started to fall. The employee stepped back then fell forward, landing on top of the plywood on the concrete floor. The employee fell on his left hip, resulting in a femoral head fracture that required hospitalization and surgery.
