An employee was helping to unload a piece of rebar from a double bender. After the programmed bend was complete, the employee approached the end of the bent bar, which was approximately 13 feet away from the point of operation, to assist with unloading the finished bar into a catch bin. As the employee approached the bar, an unprogrammed movement of the bending pin caused the bar to bend toward the employee. The rebar came loose and struck the employee, forcing him backward into a stationary conveyor table attached to the bender assembly. The employee sustained broken ribs, internal injuries, and a laceration on his cheek.
