An employee was assembling a pneumatic piston on a trim press. A crane was lifting the piston through the cavity of the press, and the employee was guiding the piston with a crowbar. The loop on the strap snapped and the piston came down. This caused the crowbar to swing forward, pinching the employee's fingers against an electrical conduit. The employee's left hand was crushed.
