Final narrative
An employee was removing the bolts from a ram door during a ram changeover. The employee loosened the bolts and then utilized a speed wrench to unscrew them from the blowout preventer (BOP). The employee then instructed the operator to open the rams. The employee noticed one of the bolts was catching on a thread and went to unscrew it the rest of the way. Their gloved left index finger was caught between the head of the bolt and the ram door that was opening resulting in a fingertip amputation.
