On October 18, 2023, at 10:15 a.m., employees were preparing to set a metal sign column onto rebar. The injured employee was getting out the rigging, steel cables, and lifting eyes to attach to the sign column. The lifting eyes are 6-by-4-inches and weigh approximately 5 to 6 pounds. As he was straightening the cables with the lifting eyes attached, one of the lifting eyes fell on his right foot. The employee suffered bruising and a subsequent infection.
