An employee was replacing the chain on an extrusion molding run table. When the machine started up again they were caught in the chain and the sprocket, resulting in an open displaced fracture to their right middle and ring fingertips, a laceration to the middle finger, and an amputation of the right index fingertip. The machine was not guarded at the time.
