An employee was using a manual pallet jack to pull a pallet of cardboard boxes off a delivery truck and onto the liftgate platform when the pallet started to descend toward the liftgate. Their foot got caught under the pallet jack wheel between the pallet and the liftgate, and moving out of the way of the descending pallet caused their foot to slip out of their shoe. They then fell and hit their head and face on the concrete sidewalk, resulting in contusions to the face and arms, a jaw fracture, and a concussion.
