The power to a shaver head was shut off, but the switch was broken and was unable to be locked out with an energy isolating device. An employee then went into the shavers area to assess a broken pull cable. As he was inspecting the cable, he stepped onto the shaver blades/plate area and his left leg was pulled into the shaver blades that were still spinning. The employee was hospitalized, and their lower left leg was surgically amputated to one inch below the knee.
