A contract employee at the Texas Concrete location was loading (hooking and unhooking) concrete pile onto a flatbed truck. The product was securely positioned when the dunnage shifted/broke causing repositioning to be necessary. A crane operator adjusted the product and the injured employee's right index finger was caught between the hook and the lifting loop of the product, resulting in a fingertip amputation.
