An employee was inside a safety cage on the forks of a forklift using an impact drill to secure a sign above the entrance in front of a new building. The cage was placed on top of the forks and the forks were angled inward. The employee was then lifted upward to complete the task. When the employee leaned in one direction, the weight distribution shifted and the cage fell to the concrete ground below. The employee sustained a collapsed lung and broken ribs. The safety cage was not attached to the forks at the time.
An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
