An employee was preparing to move a structural steel beam with a crane. As the employee started to tighten the chain and lift the beam, a dangling dog got caught on the lower part of the beam, which caused it flip over. The employee's right hand was caught between the chain sling and the beam, resulting in a partial amputation of the index finger and a fractured middle finger.
