Final narrative
An employee was measuring for insulation to patch a roof. There was a hole in the roof where an air conditioning unit had been removed. The employee was stomping the insulation over the hole to make an impression in the shape of the hole, indicating where to cut the insulation. He stepped into the hole and fell, landing on his back on the concrete floor 13 feet 3 inches below. He was hospitalized with fractured vertebrae.
