An employee was rolling a steel beam weighing 700 lbs. onto a work stand using a remote-controlled overhead crane. She disconnected a "plate dog," an attachment on the crane used to hold the steel beam, from the steel beam and was lifting it when the plate dog caught on the beam, causing it to roll off the work stand. The beam fell onto the employee's left foot, fracturing it.
