Final narrative
An employee was using a pulley system to hoist materials to the second floor. The pulley system consisted of two A-frames, a 4x4 board, a rope, and a chain around the 4x4. The rope failed and either the pulley or the hook on the end of the rope struck the employee's head, resulting in a laceration and brain bleed.
