An employee finished loading a trailer and needed a strap to secure the freight. He drove his forklift down the dock to the next sector to get a strap from another trailer. He picked up the strap and was driving back to the loaded trailer when the strap got caught under the forklift's tires. The strap was pulled from his hand and skin was removed from his right arm. He also sustained a contusion and an open wound down his right arm and wrist. The employee was hospitalized.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was operating a compact open-cab skid steer. As he backed up, the skid steer's bucket lowered onto his leg, pinning it against the body of the skid steer. He was hospitalized, requiring surgery to the ankle/lower leg.

An employee was backing up a garbage truck at a landfill to empty the truck. A concrete platform gave way, causing the vehicle to slide down into the landfill pit 20 feet below. The employee was flung backward and forward, hitting his head on the dashboard of the vehicle. The employee was hospitalized with a concussion.

An employee was operating a rubber-tracked crawler carrier to offload and place timber mats that are used for vehicular and equipment access on a utility right-of-way. The offloading of the mats from the dump bed caused the front end of the equipment to elevate and then come back down with force. The employee sustained compression fractures to two vertebrae in their lower back and was hospitalized.
