An employee was cleaning a concrete mixer after a pour had taken place by hammering the outside of the container. The employee dropped their hammer into the machine opening and their hand was caught and crushed by the mechanical paddles when they went to retrieve it. The employee was hospitalized with an open fracture of their right hand and amputation of the upper half of their left hand. The employee required surgery.
