The injured employee was removing labels from boxes. A forklift was driving to pick up boxes and the driver stopped to talk to the injured employee. After the driver spoke with the injured employee, the forklift reversed and ran over the injured employee's right foot, crushing it. The employee's foot was surgically amputated midway up the foot.
