An employee was on a scaffold installing masonry veneer when a plank on the scaffolding fell from its supports, causing the employee to fall approximately 13 feet to the ground below. The employee sustained fractured ribs and a punctured lung.

An employee was conducting pressure washing on the roof of a commercial building. The employee fell through a fiberglass skylight panel and struck the concrete pavement approximately 30 feet below. The employee sustained fractures to their pelvis, femur, and heel. The employee was using fall protection at the time of the incident, including a harness and lanyard, but it failed to arrest the fall.

Employees were installing wood roof trusses, setting them on concrete masonry unit walls that were capped with new double 2-by-6 top plate. The trusses collapsed in a chain reaction, causing three employees to fall to the slab below.
