On August 26, 2025, an employee was moving a loaded pallet with a pallet jack. She pulled on the pallet jack to get it unstuck from the pallet. When the pallet jack came loose, she lost her balance and fell on the concrete floor by the loading dock door. She landed on her right hip and it was fractured in three places. She was hospitalized and required surgery.

An employee was securing a load using a heavy-duty ratchet strap tool. The tool slipped off the ratchet, causing the employee to lose balance and fall backward onto the ground, landing on his right hip. He was hospitalized with a femur fracture.

A delivery driver was working to open a door at a supermarket so he could make his delivery of bread products. The door was stuck, and when he applied force to open it, it came loose, knocking him back and causing him to strike his head on the wall or floor. The employee sustained a skull fracture, brain swelling, and lost consciousness.

An employee was walking down the hallway when their leg gave out and they lost their balance and fell to the floor, resulting in a left hip fracture.

An employee was checking out a customer at the register. She grabbed the hand scanner and went to walk around. When she turned, she lost her balance and fell to the floor on her left side. She sustained a fractured hip and required surgery.
