An employee was using an compact utility loader to clean out mud and gravel that had washed into a 6-foot-by-9-foot box culvert. While moving toward a pile of gravel to fill the bucket, the upward/forward force caused the employee to become caught between the ceiling of the box and the top portion of the compact utility loader. The employee sustained a fractured jaw and required surgery.
