A driver had a converter dolly hooked to his tractor and was working to hook up the trailer. When he released the pintle hook, the dolly eye hook sprang up, causing a crushing injury to his left hand.

Equipment Depot - Romeoville

An employee was using a punch and hammer to remove a pin from the mast of a forklift. The mast was secured by an overhead crane. The mast shifted, and the employee's thumb was pinched between the mast and the punch, resulting in an amputation to the tip of his left thumb.

Anna Jaques Hospital

An employee was conducting maintenance on a HVAC unit. A belt was replaced. The employee then spun the belt and motor/drive assembly by hand to ensure that the belt was seated properly, and that the motor/drive assembly was in proper alignment. His right little finger became trapped between the belt and motor/drive assembly, resulting in partial amputation of the distal aspect.

M. A. Mortenson Company, Inc

An iron worker was assisting with hooking up a pickup truck to a trailer. The spring-loaded clasp that covers the hitch connection pinched his left ring fingertip, resulting in an amputation of the distal phalanx involving the tuft.

Kamco Industries Inc

At 5:00 p.m. on September 24, 2025, an employee was helping remove a fixture from a press when it slipped off the track. The employee was working to lift the fixture and set it on a mobile cart when his left middle fingertip was pinched between the fixture and a post on the cart. The employee suffered an amputation to the fingertip.

NIAGARA BOTTLING LLC

A mechanic was performing a modular overhaul on a blower. While removing magnetic rods for the stretch rod, the employee was placing the second rod on the blower platform. The rods were magnetically attracted to one another, and the employee's left third and fourth fingers were pinched between them. She was hospitalized and sustained a partial amputation.
