An employee had just removed a tray of 80 glass ampoules, each containing 1.44g of dinitrogen tetroxide (N2O4), from the burst test oven. The employee was examining the ampoules when a chain reaction caused 77 of them to burst in close proximity to his face. The released contents quickly converted into nitrogen dioxide (NO2). The employee was hospitalized with chemical burns to his face and corneal abrasions from contact with the glass.
