Final narrative
Two employees pulled a broken sheet of glass out of a machine. As they started to lift and toss the glass sheet into a bin, the glass sheet broke and a piece of glass contacted the injured employee's left forearm. The employee was hospitalized with a laceration to their ulnar artery.
