An employee was descending from a roof onto a wall-mounted roof hatch ladder. He put his left foot on the top rung, then tried to lower his right foot, at which point his left foot slipped off the ladder. He fell to the ground 10 feet below, landed on his right ankle, and suffered fractures to the tibia and fibula.
