Final narrative
An employee was re-piping an air tank. While the employee was using an 18-inch aluminum pipe wrench to remove a 3-inch pipe nipple from the tank, the wrench's handle broke. This caused the employee to fall backward and sustain a fracture to their right leg about 2 to 4 inches above the ankle.
