An employee was holding a part to be put into a press when his foot slipped and struck the foot petal. The machine cycled and pulled the part into the press, causing the employee to be pulled toward the machine. His right hand got trapped between the press and the part, resulting in four crushed fingers that required surgery and an amputation of the little finger.
