Before the incident
EE was working on the bflux line. The sewing machine failed to cut the string after the bag was finished being sewn. This pulled the string from the needle.
What happened
During the rethreading of the sewing machine the EE forgot to LOTO the machine to do this task. The machine started up after the EE bumped the start button cause the needle to penetrate the EE finger nail.
