On [REDACTED] at approximately [REDACTED] a forklift operator was picking up a roll in Laminating and inadvertently hit the roll with his forklift and tipped the roll on its side. The forklift operator subsequently attempted to manually lift the roll to set it back in an upright position. As the forklift operator attempted to lift the roll he fell forward and hit his face on the role core.
