Before the incident
The forklift operator places the hook tip onto the cable zip tie and pulls the KLEVER KUTTER disposable dual-hook safety cutter towards the left thumb.
What happened
As soon as the KLEVER KUTTER disposable dual-hook safety cutter cuts through the cable zip tie the hook tip pierces contacts the inside of the left thumb and across toward the outside of the thumb. This caused approximately one-inch laceration on the left thumb.
