Before the incident
While on ERF tuyere deck operator knocked split pipe all the way out of the tuyere. Took a temperature and sample on the furnace. Buildup had formed around the end of the tuyere.
What happened
Used long knockout bar to hit buildup and buildup broke free. This caused hand to contact with the tuyere head. Momentum from knockout bar caused greater force to be applied.
