Before the incident
Employee was building a large metal enclosure and working on building the gland-frames on a pallet to hold the enclosure.
What happened
Employee stepped into the pallet that they were installing the gland frame to while another employee was holding on to the frame legs. THe employee stepped into the pallet with her left foot and as she brought her right foot to fully enter the pallet footing her right foot caught the beam and she lost balance and fell to the ground. She extended her left hand to brace the impact and hit her right knee on the concrete floor.
