Before the incident
The associate was turning the forklift around by the line 1 tray stacker to head back to the palletizer discharge area to pick up a pallet In the process the associate was driving the lift forward The forks on the lift were shifted to slightly to one side He did not realize this so they drove the lift too close to the support beam by the line 1 palletizer The fork lift was close to top speed having approximately 20 feet to accelerate The associate flew forward and hit the frame of the lift From watching the video he had no idea he was going to the beam This cut his forehead and a small bump developed soon after The associate was wearing his bump cap The area was congested with trays pillows of buns etc in the way The floor was not wet and the area was lit properly
